Outreach and Support Officer
£23,000 per annum
TPP's client; a major national charity for, with and about carers are recruiting in their Barnet branch for an Outreach and Support Officer to provide a support service to carers in the community.
The purpose of this job is to deliver a support service to adult carers in the London Borough of Barnet. They role is responsible for providing information and emotional and practical support to local adult carers. The role will also entail signposting carers to the Carers Hub service and other relevant sources of support, and to help raise awareness of Carers' issues.
The successful candidate will work as part of a team to deliver a high quality carer support service at a range of venues across the borough, follow up referrals from across the Borough, register carers on the Centre's Carers Database and offer information and appropriate support. The successful candidate will also be required to conduct Carers Needs Assessments and reviews with carers, assist carers with completing emergency plans, make referrals on behalf of the Carer and provide information, signposting and emotional support.
To be considered for this post you will need to hold GCSE's or equivalent in Maths and English, understand carers' needs, challenges and issues, have an awareness of the roles of statutory and voluntary organisations within health and social care. You will also need to have demonstrable experience of building relationships with people from a variety of communities and backgrounds in order to develop person centred plans and find solutions.
To fulfil the duties required across the location stipulated you will need to hold a full clean UK driving license and have access to a car insured for business purposes.
