About the role
The aim of the role is to work with and support a caseload of people who have significant health barriers to employment in order to reduce these barriers and increase wellbeing and independence. This will be achieved by working in collaboration with Evolve’s service staff and the Work and Learning Team, to identify customers who are suitable for the Croydon Health Employment Partnership. This includes the provision of one-to-one support to customers to identify and address health barriers and to achieve skills and employment outcomes.
As well as having experience of working in a homeless or other support service, you must have a clear understanding of why people become homeless, and the needs and challenges they face.
You must have an understanding of the employment and health barriers that homeless people face and a knowledge of the relevant pathways. You will be required to take on a caseload of people, supporting them to achieve positive outcomes through the use of workshops and tailored one to one support.
Working as part of the Work and Learning team you will work in partnership with other teams within Evolve, as well as with CHEP partners, to help sustain and develop the partnership. You must be able to communicate clearly, both verbally and in writing, and be competent in the use of Microsoft Office applications.

-To provide one to one support to customers to identify and address health barriers and to achieve skills and employment outcomes.
-To maintain accurate records of support provided and record related information on skills and employment outcomes.
-Work with and manage a caseload of people who face significant health barriers to employment.
-Develop individual plans with customers that address health barriers and identify aims and aspirations.
-Provide one to one support to customers with the aim of developing independence around their health.
-Provide one to one support to customers to identify their aims and aspirations and set realistic plans for how to acheieve these .
-Provide support and guidance to customers to develop skills and increase employability .
-Liasie with community services and health sevices to enable cusomters to access healthcare and better manage their health.
-To work in partnership with the Work and Learning Team, external agencies and CHEP partners to identify and access opportunities for training, volunteering and employment.
-To work alongside CHEP partners to develop the work of the partnership, share good practice, ensure monitoring is complete and to hold workshops and events.
-Input customer outcomes and records into InForm, Evolve’s customer records database.
-Undertake regular monitoring and evaluation activities and produce data in order to measure outcomes.
